Dead Space series. It's gory. It's nasty. And it's violent. Disgusting, slimy aliens feel too real sometimes. Alien fatalities are disturbing. Great game, I'd say, but I don't have the balls (I admit) to finish the series. :)

Also, Manhunt series. Yeah, the second instalment's graphics ain't that good - but the fatalities (execution moves) are much  bloodier than Mortal Kombat series combined together. ::)

The Thing? it's after a horror movie  :o
I have the retail of the game for 5 years and yet i have no guts to finish it.TOO SCARY  :'(

Surprised the Punisher didn't make the cut for the top ten, I know for out right gore it was maybe not the worst but some of the scenarios were kinda horrifying (head first into a shredder? lol!)

It almost did, the other games were slightly nastier though.  Plus the real Punisher game doesn't actually show that stuff, you need patches and mods ect.

The problem I usually face when asking the question of what the most gory PC game is is that I've basically played every sick, twisted, gore-filled game.

I've played both the Manhunts (Manhunt 2 on PC is UNCENSORED!), all the Postal games, all the Soldier of Fortune games, The Punisher and most other titles that have a fair share of gore in them.

I'd recommend Manhunt 2 on PC for anyone who really enjoys violence and gore. I've played through it twice on PC and Rockstar Games was gracious enough to provide PC gamers with the uncensored AO version of the game.  ;D
